Reason for return:
Part and Serial Numbers:
In order to issue an RMA we require the part number for each unit along
with the corresponding serial number. Both numbers can be found
on the product label. Examples would include:
Part Number 1749A-13795 Serial Number 000033520
Part Number 628A02TBE Serial Number 95284122A
If the part does not have a serial number please enter "N/A"
If either number is illegible, please state that the number cannot be read
